House of Slay LLC– Lash & Hairstyling Services

1. General

By booking an appointment or receiving services from House of Slay LLC you agree to comply with and be bound by the following Terms and Conditions. These terms apply to all clients receiving lash and hairstyling services.

2. Appointments & Booking

• All appointments must be scheduled in advance via [booking platform/contact method].

• A valid payment method may be required to secure your appointment.

• Please arrive on time. Late arrivals may result in reduced service time or cancellation.

3. Deposits

• A non-refundable deposit of up to $100 may be required to book certain services.

• Deposits will be applied toward your final service total.

• Deposits are forfeited for cancellations that violate the cancellation policy.

4. Payments Accepted

We accept the following forms of payment:

• Debit Cards

• Credit Cards

• PayPal

• Cash App

Payment is due in full at the time services are rendered. No checks are accepted unless otherwise stated.

5. Cancellation & Rescheduling Policy

• A minimum of 24 hours’ notice is required to cancel or reschedule an appointment.

• Cancellations made less than the required notice period may result in:

◦ Loss of deposit

◦ A cancellation fee of up to $50

• No-call/no-show appointments will be charged at 60% of full appointment price and may require full service price prepayment for future bookings.

6. Refund Policy

• All services are final and non-refundable.

• If you are dissatisfied, please notify us within 24 so we can address concerns and offer a possible correction.

• Corrections are only offered at the discretion of the business.

7. Service Policy

• Results may vary depending on hair type, lash condition, and aftercare.

• Clients must disclose any allergies, sensitivities, or medical conditions prior to service.

• We reserve the right to refuse service if there are concerns regarding safety, hygiene, or unrealistic expectations.

8. Late Policy

• Clients arriving more than 15 minutes late may be required to reschedule.

• Late arrivals may incur a fee or shortened service time.

9. Sanitation & Safety

• All tools and equipment are sanitized according to industry standards and Georgia state regulations.

• Clients must arrive clean and prepared for services (e.g., no eye makeup for lash services unless otherwise instructed).

10. Liability Waiver

By receiving services, you acknowledge:

• There are inherent risks associated with lash extensions and hairstyling services.

• House Of Slay LLC is not liable for allergic reactions or complications if proper information was not disclosed prior to service.

• You agree to follow all aftercare instructions provided.

11. Chargebacks & Payment Disputes

• By providing your payment information, you agree not to initiate chargebacks or disputes without first contacting us to resolve the issue.

• Fraudulent disputes may result in legal action and refusal of future services.

12. Children & Guests

• For safety and insurance reasons, no extra guests or children are allowed unless prior approval is given.

13. Right to Refuse Service

We reserve the right to refuse service to anyone for reasons including, but not limited to:

• Inappropriate behavior

• Non-compliance with policies

• Health or safety concerns

14. Privacy

Client information is kept confidential and used solely for appointment scheduling, communication, and internal records.

15. Changes to Terms

We reserve the right to update these Terms and Conditions at any time. Continued use of our services constitutes acceptance of any changes.
